- Separate issue from ambiguity. Every defect receives a stable number and dated visual record.

- Track phased access. Keep open work visible across wards, departments and plant areas without flattening it into one spreadsheet.
- Evidence completion. Add the after photo to the same record and retain what was originally reported.
